Why Men Die Younger Than Women in Almost Every Country on Earth
Men die 5 years younger than women on average, and in some countries the gap is over 10 years. The reasons are biological, behavioural, and systemic.
A few months ago I was looking at life expectancy data for a project and noticed something I had never really thought about before. In every single country I checked — rich or poor, peaceful or war-torn, tropical or arctic — women outlived men. Not by a little. By years. I checked 195 countries and could not find a single exception. Not one. That felt like something worth understanding, so I pulled the numbers apart.
What I found is that the gap between male and female lifespan is not a coincidence, and it is not just one thing. It is biology, behaviour, culture, and systems all compounding together into a pattern so consistent it shows up on every continent.
The headline number
Globally, women live to an average of 73.8 years. Men live to 69.0 years. That is a gap of 4.8 years — and in some countries it stretches past a decade.
To be clear: this is not a small statistical artefact. It is nearly five years of life, averaged across 8 billion people. Five years of retirement you do not get. Five years of grandchildren you do not see. Five years of mornings.

Europe has the widest regional gap — 6.4 years — driven largely by Eastern European countries where male mortality from alcohol and cardiovascular disease is extremely high. Africa has a narrower gap, but for a grim reason: women there face significant maternal mortality and reduced healthcare access, which pulls their life expectancy down toward the male average rather than the male average rising.
The countries where the gap is widest
Some countries have a gender life expectancy gap that is genuinely staggering.

The post-Soviet countries dominate this list, and that is not a coincidence. The collapse of the Soviet Union in the 1990s triggered an epidemic of alcohol abuse, unemployment, and despair among men — particularly working-age men — that caused male life expectancy in Russia to drop below 60 at its worst point. It has recovered somewhat, but the scars remain. Alcohol alone is estimated to account for more than half of premature male deaths in Russia.

Where the gap is smallest
On the other end of the spectrum, some countries have a gap of barely two years.

A small gap is not necessarily good news. In several of these countries, the gap is narrow because women die younger than they should — not because men live longer. High maternal mortality, limited access to healthcare during pregnancy, gender-based violence, and discrimination in who receives medical care all pull women's life expectancy down. In Chad, both sexes die young. The gap being small does not mean anyone is doing well.
The biology: women are built to last
Part of the answer is simply biological. Women have structural advantages that are baked into their genetics.
Estrogen is cardioprotective. Before menopause, estrogen helps maintain healthy cholesterol levels, keeps blood vessels flexible, and reduces inflammation. This is a significant part of why cardiovascular disease kills men earlier and more often than women. After menopause, when estrogen drops, women's heart disease rates begin to catch up — but they start from a much lower baseline.
Two X chromosomes provide a genetic backup. Women carry two copies of the X chromosome. If one copy has a harmful mutation, the other can compensate. Men have one X and one Y — there is no backup. This is why conditions like haemophilia and Duchenne muscular dystrophy are overwhelmingly male diseases.
Testosterone encourages risk. This is both behavioural and biological. Testosterone is associated with higher aggression, greater risk-taking, and reduced pain sensitivity. These traits had evolutionary advantages. In a modern world full of cars, alcohol, and heavy machinery, they are liabilities.
Women have stronger immune systems. Oestrogen enhances immune function, which is why women generally fight off infections more effectively. The flip side is that women are more prone to autoimmune diseases — their immune systems are sometimes too aggressive. But in terms of raw survival from infectious diseases, women have the edge.
The behaviour: what men do differently
Biology explains perhaps a third of the gap. The rest is behaviour — and this is where the numbers get stark.

Men smoke nearly five times as much as women. They drink four times as heavily. They die on the roads three times as often. They kill themselves at 3.4 times the rate. And they die at work almost eleven times more frequently.

What kills men vs what kills women
The actual diseases that kill each sex are broadly similar — heart disease and cancer dominate for everyone — but the timing and rates are dramatically different.

Lung cancer kills more than twice as many men as women — a direct reflection of smoking rates. Road injuries kill three times as many men. Liver disease, driven heavily by alcohol, kills twice as many. And suicide takes the lives of 2.7 men for every woman.
Heart disease deserves special attention. Men develop it roughly 7 to 10 years earlier than women. A man in his mid-forties having a heart attack is common. For women, the typical onset is mid-fifties to sixties. That decade of difference, largely thanks to estrogen, is a major reason women outlive men.

The age-specific mortality gap
The gap between male and female death rates is not constant across life. It peaks sharply in young adulthood, narrows in childhood, and widens again in middle age.

Look at the 15-29 age group. Men in that bracket die at 2.5 times the rate of women. That is the age where road accidents, violence, and risk-taking behaviour peak. It is also where the "invincibility complex" is strongest — the belief that nothing bad can actually happen to you. It can. It does. Disproportionately to young men.
The workplace factor
This one gets overlooked constantly. Globally, 92% of workplace fatalities are men. Not because of discrimination in reporting — because men overwhelmingly work in the jobs that kill people.
Construction. Mining. Commercial fishing. Logging. Roofing. Long-haul driving. Oil rigs. These industries have death rates that are orders of magnitude higher than office work, and they are almost entirely staffed by men. In the UK, 97% of workplace deaths are male. In the US, it is 92%. This is not a statistical quirk — it is tens of thousands of men dying on the job every year.
The "man up" problem
Here is perhaps the most fixable part of the gap: men are far less likely to seek medical help.
Studies consistently show that men visit doctors less frequently, delay seeking help for symptoms, are less likely to attend screenings, and are far less likely to discuss mental health problems with anyone — professional or otherwise. In the UK, men are 50% less likely than women to visit their GP in any given year. By the time many men see a doctor, conditions that were treatable early have become advanced.
Prostate cancer is a good example. It is highly treatable when caught early — five-year survival rates exceed 95% for localised disease. But because men avoid screenings and ignore symptoms, a significant number of cases are diagnosed late, when survival rates drop dramatically.
The cultural pressure to "tough it out" is not toughness. It is a survival disadvantage. And it kills men in numbers that are hard to quantify precisely but almost certainly add years to the gap.
The gap is narrowing — slowly
There is a historical trend worth noting. In most developed countries, the gender gap in life expectancy peaked in the 1970s and 1980s and has been slowly closing since.

The gap peaked at 6.3 years in 1970 and has fallen to 3.6 years. The main driver is that men have been smoking less and getting better cardiac care. At the same time, the gap narrowed partly because women started smoking more in the 1960s and 1970s — those deaths are still working through the system now.
This pattern holds across most of Western Europe, North America, and East Asia. The gap is closing, but five decades of narrowing have only reduced it by about 40%. Complete closure, if it happens at all, is likely generations away.
So why does this matter?
It matters because 4.8 years is not inevitable. A portion of the gap is biological and probably cannot be eliminated. But the behavioural portion — the smoking, the drinking, the refusal to see a doctor, the dangerous driving, the untreated mental health crises — that portion is a choice. Not always an individual choice, but a cultural and systemic one.
Men do not have to die five years earlier. But right now, they do. In almost every country. Without exception.
