My mother got the phone call on a Tuesday afternoon. Her sister — my aunt — had been diagnosed with breast cancer. Stage three. I remember the silence in the kitchen after she hung up. Not crying, not panic, just this heavy, airless silence. My aunt survived. She is one of the lucky ones. But that phone call changed how I looked at the word "cancer" forever, because suddenly it was not a thing that happened to strangers in hospital wards. It was sitting at our kitchen table.

That was years ago. Since then I have spent a lot of time looking at the actual numbers, and I am going to be honest — the scale of what cancer does to the human population every single day is difficult to process, even when you lay it out in a spreadsheet.

So let me try to lay it out clearly.

The headline number

Cancer kills approximately 10 million people per year worldwide. That figure comes from the WHO and the International Agency for Research on Cancer (IARC), and it makes cancer the second leading cause of death globally, behind only cardiovascular disease.

Which cancers kill the most people

Not all cancers are created equal. Some grow slowly and respond well to treatment. Others are fast, aggressive, and almost always fatal by the time symptoms appear. The gap between the deadliest and least deadly cancers is enormous.

Lung cancer is the single biggest killer — and it is not close. It accounts for nearly one in five cancer deaths worldwide. The connection to smoking is overwhelming: roughly 80% of lung cancer deaths are attributable to tobacco use. Which countries lose the most people to cancer

Cancer death tolls concentrate in countries with large, ageing populations. China alone accounts for nearly a third of all global cancer deaths — driven by high rates of lung, liver, and stomach cancer.

Notice the pattern in the last column. Lung cancer is the top cancer killer in every single one of these countries. That is not a coincidence — it is the direct, measurable consequence of global tobacco use over the past century.

The survival rate gap

Here is where the data gets really stark. Some cancers, caught early, have excellent survival rates. Others are a near-certain death sentence regardless of when they are found.

A woman diagnosed with breast cancer has a 90% chance of being alive five years later. A person diagnosed with pancreatic cancer has a 12% chance. That is the difference between a disease you fight and a disease that fights you.

My aunt fell into the 90% column. Many people do not. And the distribution is not random — it depends on the type, the stage at diagnosis, the country you live in, and whether you can access treatment. A breast cancer patient in the UK or US has radically different odds than one in sub-Saharan Africa.

The age factor

Cancer is often thought of as an old person's disease. That is mostly true — but the "mostly" matters.

About 77% of cancer deaths occur in people aged 60 and over. That is expected — cancer risk increases with age because DNA accumulates more mutations over time, and the immune system weakens.

But look at that first row. One hundred thousand children under 15 die of cancer every year. That is 274 children per day. Leukaemia, brain tumours, and lymphomas are the most common childhood cancers. They are relatively rare compared to adult cancers, but for the families involved, rarity is no comfort.

The 40–59 age group also deserves attention. 1.7 million people per year dying of cancer in what should be the productive middle of their lives. These are parents, workers, people with decades ahead of them — cut short.

What causes cancer — and how much is preventable

This is the part people find hardest to sit with. The WHO estimates that 30–50% of all cancers are preventable through lifestyle changes and public health interventions. That means somewhere between 3 and 5 million of those 10 million annual deaths did not have to happen.

Tobacco is the single largest preventable cause of cancer on Earth. Over 2 million cancer deaths per year trace directly back to smoking. That is more than the next three risk factors combined.

Infections are the one that surprises most people. Over a million cancer deaths per year are caused by infections that we either already have vaccines for (HPV, Hepatitis B) or can treat with antibiotics (H. pylori). Cervical cancer — which kills over 340,000 women per year — is almost entirely caused by HPV and is almost entirely preventable with vaccination. The fact that hundreds of thousands of women still die from it every year is not a medical failure. It is a distribution failure.

Roughly 11,000 people die every day from cancers that could have been prevented. That is the equivalent of a small town wiped out daily by causes we already understand and, in many cases, already know how to stop.

The "genetic or unknown cause" category includes people who did everything right — never smoked, ate well, exercised, had no family history — and got cancer anyway. Sometimes the body simply makes a mistake during cell division. One error in three billion base pairs and a normal cell becomes a cancer cell. No amount of kale or morning runs can eliminate that risk entirely.

The global trend

Cancer deaths are rising. Not because cancer is getting worse, but because the world population is growing and ageing. More people living longer means more people reaching the age when cancer becomes common.

By 2040, cancer is projected to kill over 16 million people per year. That would be roughly 44,700 per day. The increase is driven almost entirely by population growth and ageing in low- and middle-income countries, where access to screening and treatment lags decades behind wealthier nations.

What you can actually do

I am not going to give you a list of superfoods or tell you to meditate. But the data does point to a handful of things that meaningfully reduce cancer risk:

  • Do not smoke. If you do, stop. This single change eliminates the largest preventable risk factor for cancer. Period.
  • Limit alcohol. There is no safe level for cancer risk. Less is better.
  • Get screened. Breast, cervical, and colorectal cancer screening catches cancers early when survival rates are highest.
  • Get vaccinated. HPV and Hepatitis B vaccines prevent the infections that cause liver and cervical cancer.
  • Maintain a healthy weight. Obesity is linked to at least 13 types of cancer.
  • Wear sunscreen. Melanoma is one of the most preventable cancers.

Tying it together

About 90 of those deaths were preventable. People who might still be alive if they had never picked up a cigarette, if HPV vaccination had reached their village, if screening had caught the tumour six months earlier.

Cancer is not one disease. It is hundreds of diseases wearing the same name, each with different causes, different survival rates, and different levels of preventability.
