Here is the part that surprises most people: heart disease and stroke kill more smokers than lung cancer does. This part rarely gets discussed, but it matters. Tobacco-related deaths do not just affect the people who die. They affect families, workplaces, and healthcare systems.

That $1.4 trillion figure comes from the WHO. To put it simply, the global economy loses over a trillion dollars every year because of smoking — through medical bills, lost working years, and the reduced productivity of people too ill to work at full capacity.

The trend: it is falling, but slowly

Here is the one bit of good news buried in all this data: smoking rates are declining worldwide. They have been declining for decades in most high-income countries, and more recently in middle-income countries too.

Wait — if the smoking rate is falling, why are deaths still rising? Because there is a lag. The diseases that tobacco causes — COPD, lung cancer, cardiovascular disease — take 20 to 30 years to develop. The people dying today started smoking in the 1990s and 2000s. The decline in smoking rates will eventually show up as a decline in deaths, but not for another decade or two.

It is like turning a massive ship. You can see it starting to turn, but the momentum carries it forward for a long time before it actually changes direction.

What about vaping?

Vaping is too recent for us to have reliable long-term mortality data. Nobody has been vaping for 30 years, which is the kind of timeframe you need to measure whether something causes cancer or COPD. What we do know is that e-cigarettes contain fewer toxic chemicals than traditional cigarettes, but "fewer" does not mean "none." Nicotine itself raises blood pressure and heart rate, and several of the flavouring chemicals used in vapes have been linked to lung damage in laboratory studies.

The honest answer is: we do not know yet how many people vaping will eventually kill. The data does not exist. Anyone who tells you it is completely safe is guessing. Anyone who tells you it is as dangerous as smoking is also guessing.
