"Cardiovascular disease" is an umbrella term. It covers several distinct conditions, all involving the heart or blood vessels.

Russia stands out with the highest death rate per capita among large countries — 750 per 100,000, more than double the rate in the US or UK. Russian men have a life expectancy of just 66, and cardiovascular disease is the primary reason. China and India dominate the absolute numbers simply because of population size. Together, they account for more than 40% of all cardiovascular deaths on Earth.
In sub-Saharan Africa, the CVD death rate appears lower, but that is partly because people die younger from infectious diseases before they reach the age where cardiovascular disease typically strikes. As those countries develop and infectious disease deaths fall, CVD rates are rising rapidly. It is the next wave.
The silent killer problem
Here is the statistic that haunts cardiologists: for roughly 50% of men and 64% of women who die suddenly of cardiovascular disease, there were no prior symptoms. Their first symptom was death.
This is what makes cardiovascular disease fundamentally different from most other killers. Cancer usually announces itself — a lump, unexplained weight loss, persistent pain. Diabetes shows up in blood tests years before it becomes dangerous. But atherosclerosis — the slow buildup of plaque inside your arteries — progresses silently over decades. You can have arteries that are 70% blocked and feel perfectly fine. You can pass a stress test. You can run marathons. And then one morning, a plaque ruptures, a clot forms, and your heart stops receiving blood.

Risk factors — and how much each one matters
The risk factors for cardiovascular disease are well studied. What is less well known is how dramatically they stack.

High blood pressure is the single largest contributor. One in four adults on Earth has hypertension, and most of them do not know it. It has no symptoms until it causes a stroke or heart attack. This is why doctors call it "the silent killer within the silent killer."
The diabetes connection deserves special attention. Diabetics are two to four times more likely to die of cardiovascular disease than non-diabetics. When a diabetic dies of a heart attack, the death certificate says cardiovascular disease — but diabetes created the conditions.
Alcohol and air pollution are risk factors that people tend to underestimate. Air pollution alone contributes to roughly 3 million cardiovascular deaths per year — nearly as many as stroke.

Age and gender
Cardiovascular disease is heavily concentrated in older age groups, but it is not exclusively a disease of the elderly.

The gender gap is striking. Below age 60, men die of cardiovascular disease at roughly three times the rate of women. Oestrogen appears to provide some protective effect on blood vessels, which is why the gap narrows sharply after menopause. By age 75, the rates are nearly equal. Why cardiovascular disease dominates — the evolutionary mismatch
There is a deeper question buried in the data. Why does one category of disease kill more people than everything else on the planet?
The answer is evolutionary mismatch. The human cardiovascular system evolved for a world of constant physical movement, seasonal food scarcity, and diets built around plants, lean game, and wild fish. It did not evolve for a world of office chairs, drive-throughs, and 24-hour access to refined sugar and seed oils.
Our ancestors rarely lived long enough for atherosclerosis to kill them — infectious disease, predation, and childbirth took them first. Modern medicine eliminated most of those threats, which is an extraordinary achievement. But it also exposed the vulnerability that was always there: our blood vessels were not designed for 80 years of sedentary, calorie-dense living.
This is why cardiovascular disease is highest in middle-income countries that are transitioning from traditional to modern lifestyles. Their populations are now living long enough to develop atherosclerosis, but their healthcare systems have not yet caught up with prevention and treatment. It is the same transition that Europe and North America went through in the mid-20th century — but happening faster and at a larger scale.
What the data says actually works
The good news is that cardiovascular disease is one of the most preventable causes of death. Countries that have invested in prevention have seen dramatic results.
The United Kingdom cut its cardiovascular death rate by roughly 50% between 2000 and 2020 through a combination of smoking bans, statin prescribing, hypertension screening, and public health campaigns. The US, Australia, and most of Western Europe have seen similar declines. These are not marginal improvements. They represent millions of lives saved.
At an individual level, the evidence is remarkably clear:
- Not smoking reduces CVD risk by 50% within 2 years of quitting
- 150 minutes of moderate exercise per week reduces risk by 30 to 40%
- Controlling blood pressure below 130/80 reduces risk by 25 to 30%
- Statin therapy for high-risk individuals reduces risk by 25 to 35%
- A Mediterranean-style diet reduces risk by 25 to 30%
- Maintaining healthy weight reduces risk by 20 to 30%
None of these are exotic interventions. They are boring, unglamorous, and they work. The problem is not that we do not know what prevents cardiovascular disease. The problem is that prevention requires sustained behaviour change, and sustained behaviour change is the hardest thing in medicine.
