Two babies are born at the exact same moment. One arrives in a hospital in Tokyo, the other in a clinic in N'Djamena. Same species, same biology, same number of chromosomes. But the baby in Tokyo will, on average, live to see 84. The baby in Chad will not make it to 53. That is a 32-year gap — an entire generation — determined by nothing more than geography.
It is one of those facts that, once you really sit with it, changes how you think about everything. Luck of birth is the single biggest predictor of how long you will live. Not your genes. Not your willpower. Not your diet. Where you happened to be born.

These are the countries where people live the longest. The list is dominated by wealthy nations with universal healthcare, low violence, and strong sanitation infrastructure.

Japan has held the top spot for decades. Its combination of diet (fish-heavy, low in processed food), universal healthcare, high social cohesion, and extraordinarily low rates of violent death make it the global benchmark for longevity. Okinawa alone has been studied for years as a so-called "Blue Zone" where centenarians are unusually common.
Notice something about this list: wealth alone does not guarantee a spot. The United States, the richest country in history, does not crack the top 20. It sits around 77.5 years — dragged down by gun violence, opioid deaths, obesity, and a healthcare system that leaves millions uncovered. Money matters, but how a society distributes it matters more.

Every single country in the bottom 20 except Afghanistan is in sub-Saharan Africa. The average life expectancy across these nations is about 56.8 years — meaning the typical person dies before what most high-income countries would consider retirement age.
The causes are layered: inadequate healthcare infrastructure, high infant and child mortality from preventable diseases, limited access to clean water, malaria, tuberculosis, HIV/AIDS, and in many cases, active armed conflict. What determines how long you live
Life expectancy is not random. It is shaped by a handful of measurable forces, and each of them shows up clearly in the data.
Healthcare access and quality
Countries with universal healthcare systems consistently outperform those without. This is not just about treating sick people — it is about catching disease early, vaccinating children, providing prenatal care, and making sure a heart attack at 50 does not become a death sentence because the patient cannot afford an ambulance.
The Healthcare Access and Quality Index (HAQ), scored out of 100, correlates almost perfectly with life expectancy:

Sanitation and clean water
Roughly 1.4 million people die each year from diarrhoeal diseases — overwhelmingly in countries without reliable access to clean water. This is not a disease of biology. It is a disease of infrastructure. Fix the pipes, and people stop dying.
Diet and lifestyle
Japan, Spain, and Italy share something beyond good healthcare: their traditional diets are rich in vegetables, fish, olive oil, and whole grains. They also have relatively low rates of obesity. Meanwhile, countries with high consumption of processed food, sugar, and alcohol see significantly shorter lifespans.

GDP per capita

The relationship is not linear — it flattens sharply above about $25,000. Going from $5,000 to $25,000 per capita adds roughly 11 years of life expectancy. Going from $25,000 to $50,000 adds only 4.3 more. And above $50,000, the gains are marginal. This tells us something important: the basics matter enormously, the luxuries less so.
Violence and conflict
Countries with active armed conflicts — Afghanistan, South Sudan, Somalia, Yemen — see life expectancy drop by 5 to 15 years compared to peaceful neighbours with similar income levels. War does not just kill soldiers. It destroys hospitals, disrupts food supply chains, and displaces millions into conditions where disease spreads rapidly.

Africa's average is 15 years below Europe's. That is not a rounding error — it is the difference between meeting your grandchildren and not meeting them. Between retiring and never retiring. Between seeing the world change and being taken out of it while your children are still young.
The gap between Africa and the rest has been narrowing, slowly, since the early 2000s — driven primarily by HIV/AIDS treatment rollouts and childhood vaccination programmes. But progress has been uneven, and COVID-19 set some countries back by years.

In 1950, the average human could expect to live 46.5 years. Today it is 73.3. That is an increase of nearly 27 years — more than half a lifetime — in just three generations.
The drivers of this improvement are remarkably unglamorous: clean water, childhood vaccines, antibiotics, better sanitation, and the near-elimination of famine in most of the world. No single miracle drug or breakthrough surgery did this. It was plumbing and penicillin.
The rate of improvement is slowing, though. The 1950-to-1970 jump of nearly 10 years happened largely because infant mortality collapsed in developing countries. Those easy gains have been captured. The remaining improvements require tackling harder problems — chronic disease, ageing populations, and inequality in healthcare access.

The gender gap
In virtually every country on Earth, women live longer than men. The global average gap is about 4.5 years, but it varies widely.

Russia has the largest gender gap in the developed world — 10.2 years. Russian men die younger largely because of extraordinarily high rates of alcohol abuse, smoking, and cardiovascular disease. A Russian man born today has a lower life expectancy than men in Bangladesh or North Korea.
The biological advantage women hold is real — oestrogen appears to protect against cardiovascular disease before menopause, and women have two X chromosomes providing genetic redundancy against certain inherited conditions. But the gap is also behavioural. Men are more likely to smoke, drink heavily, take physical risks, die in violence, and avoid seeing a doctor.

The data shows one thing clearly: the biggest determinants of longevity are things societies choose — investment in healthcare, clean water, food systems, and peace. Individual choices matter too. But no amount of personal discipline can fully overcome the disadvantage of being born in the wrong place. A health-conscious person in Sierra Leone will still, on average, die younger than a chain smoker in Norway. That is the uncomfortable truth at the heart of all this data.
Where does this leave us
The good news is that the gap is closing. Life expectancy in Africa has risen by nearly 12 years since 2000. Childhood mortality has been cut in half globally since 1990. Diseases that killed millions — measles, malaria, cholera — are increasingly preventable and treatable.
The bad news is that new threats are emerging. Obesity, sedentary lifestyles, antibiotic resistance, and climate-driven disease are all pushing in the wrong direction. And in some wealthy countries, including the United States and the UK, life expectancy has stagnated or even dipped in recent years.
