On top of the 1.35 million killed, between 20 and 50 million more people are seriously injured in road accidents each year. Many of those injuries are permanent — spinal damage, amputations, traumatic brain injuries. The death toll is only part of the picture.

Who is dying?

Road deaths are not distributed evenly across the population. They cluster around certain demographics in ways that are worth understanding.

Three out of four road deaths are men. This holds true across almost every country in the world and every age group. Men drive more, drive faster, take more risks, and are significantly more likely to drive under the influence. It is not a cultural stereotype — it is a global statistical reality.

The other thing that stands out is motorcyclists. They account for 28% of all road deaths despite making up a tiny fraction of total road users. In Southeast Asian countries where motorcycles are the primary form of transport, this figure is even higher.

Road accidents are the leading cause of death for people aged 15 to 29 worldwide. Not cancer. Not heart disease. Not suicide. Car crashes. For young people, the road is the single most dangerous thing they encounter in daily life.

After age 60, the pattern shifts — older people are more likely to die as pedestrians, often because slower reflexes and reduced mobility make crossing roads more dangerous.

Libya's road death rate is 36 times higher than Norway's. A Libyan is 36 times more likely to die on the road than a Norwegian, on a per-capita basis. That is not a small difference — it is an entirely different reality.

What actually causes these deaths?

People assume most road deaths are caused by dramatic, high-speed collisions. Some are. But the data tells a more nuanced story.

Speeding is the single biggest killer, involved in roughly 30% of all fatal crashes. But here is the thing most people do not appreciate: the relationship between speed and death is not linear. It is exponential. A pedestrian hit at 30 mph has a roughly 10% chance of dying. At 40 mph, that jumps to 40%. At 50 mph, it is 80%.

This is why a 20 mph zone in a city centre is not just bureaucratic caution — it is the difference between a bruised pedestrian and a dead one.

The economic cost

The global cost of road crashes is approximately $1.8 trillion per year. That is more than the entire GDP of Canada. And the burden falls disproportionately on the countries least able to afford it — low-income countries lose 5% of their GDP to road crashes, more than three times the proportion lost by wealthy countries.

This includes medical costs, emergency services, lost productivity, property damage, and the long-term care of people with permanent injuries. It does not include the immeasurable human cost of grief.

The trend: slowly improving, but not fast enough

The per-capita death rate is declining slowly — from 18.9 to 16.5 per 100,000 over 25 years. But total deaths have stayed roughly flat because the world's population has grown and more people are driving. In low- and middle-income countries, total road deaths are still rising.

The 2020 dip is a COVID effect — lockdowns meant fewer cars on the road. Once restrictions lifted, deaths bounced right back.

What actually works

Decades of data from countries that have dramatically reduced their road deaths show a clear pattern. It is not one thing — it is a system of interventions applied together.

  • Speed limits and enforcement — every 1% reduction in average speed produces a 4% reduction in fatal crashes
  • Seatbelt laws — seatbelts reduce the risk of death for front-seat passengers by 45-50%
  • Drink-driving laws — countries with strict enforcement see 20-30% fewer alcohol-related deaths
  • Motorcycle helmet laws — helmets reduce the risk of death by 42% and head injury by 69%
  • Road infrastructure — roundabouts instead of intersections, barriers between lanes, better lighting, pedestrian crossings
  • Vehicle safety standards — airbags, electronic stability control, autonomous emergency braking

Sweden pioneered the "Vision Zero" approach — the idea that no death on the road is acceptable, and that road systems should be designed to protect humans from their own mistakes. Since adopting it in 1997, Sweden has cut its road death rate by more than half.

Putting it in context

Road deaths kill 1.35 million people per year. That is more than malaria (620,000), more than HIV/AIDS (630,000), and nearly double the number killed by suicide (700,000). It is the eighth leading cause of death globally, and the number one killer of young people.
